We measure our outcomes using various indicators, such as member satisfaction, quality of care, service utilization, cost savings, population health improvement and community benefit.

Member Satisfaction

We value the feedback and opinions of our members and their families. We conduct regular surveys and interviews to assess their satisfaction with our services and programs. We use that input to improve our quality and performance. In the fiscal year 2022, we achieved a 90% member satisfaction rate across our services and programs. This reflects our commitment to providing personalized and integrated care that meets the physical, emotional, and social needs of each person we serve.

Quality of Care

We strive to provide the best possible health outcomes for our members and the communities we serve through innovation, integration, and empowerment. We use evidence-based practices and data-driven decision making to ensure our services and programs are effective and efficient. We also monitor and evaluate our quality and performance using various measures, such as clinical indicators, accreditation standards, regulatory requirements, and contractual obligations. In the fiscal year 2022, we met or exceeded most of our quality targets across our services and programs. Some of our achievements include:

Service Utilization

We strive to provide the right service at the right time at the right place for our members. We use a person-centered approach to identify their needs, strengths, preferences, and goals. We also use a service plan to outline the type, frequency, and duration of services they will receive. We coordinate with other providers and partners to ensure continuity and integration of care. In the fiscal year 2022, we served over 30,000 members across the state of Arizona. We also expanded our integrated health clinics to six locations and opened a new clinic in Lakeside, Arizona.

Cost Savings

We seek to provide cost-effective services that deliver value for our members, funders and partners. We use various strategies to reduce costs and increase efficiency, such as leveraging economies of scale, optimizing resource allocation, streamlining processes and systems, implementing best practices and innovations, and negotiating favorable contracts and agreements. In the fiscal year 2022, we generated over $10 million in cost savings for our members, funders, and partners. Some of our cost-saving initiatives include:

Population Health Improvement

We aspire to improve the health, well-being, and quality of life of our members and the communities we serve through prevention, intervention, and promotion. We use a population health approach to address the social determinants of health that affect our members and communities, such as income, education, housing, employment, environment, and social support. We also use a community engagement approach to involve our members and communities in designing, implementing, and evaluating our services and programs. In the fiscal year 2022, we made considerable progress in improving the population health of our members and communities. Some of our population health improvement programs include:

Community Benefit

We endeavor to contribute to the social good of our members and the communities we serve through philanthropy, advocacy, and leadership. We use a social responsibility approach to address the needs and challenges of our members and communities, such as access, equity, diversity, and inclusion.
